Special Request Form for Financial Assistance
Our concern for you is not limited to your financial situation. We care about your emotional, spiritual, and general well-being also. We understand, however, that difficult or unexpected circumstances arise from time to time so we try to accommodate our time, services, or financial assistance if and when additional resources are available. Although you are not required to pay back the money, we would like for you to strongly consider repaying the gift so that it can be used to help someone else in need.
Due to various requests for help or sponsorships received we have developed some general guidelines to help us determine when or how we may be of assistance to you. If we’re not able to assist you, we will inform you accordingly and/or refer you to other agencies as an alternative. Please be advised that assistance is not guaranteed! We require ample time to review all requests so that we adequately support the need. Failure to provide requested information will result in forfeiture or delay your request.
Guidelines:
1. Highest priority will be given to Johnston County recipients
2. Assistance will be provided in form of goods, services or financial assistance.
3. The type depends on the requestor and/or availability of funds.
4. Recipient may not apply on behalf of someone else.
5. No funding will be given directly to the applicant, but to the agency, vendor, business, or service provider where the amount is needed if approved.
6. Only checks will be distributed (no cash)
7. Financial assistance will only cover basic needs
8. Only completed forms will be reviewed
9. Final decision may take up to 7 business days
10. Requestors must provide explanation for which funds will be used.
